Vet Visit
A vet has ten to fifteen minutes and no memory of the last three weeks in your house. These forms exist to close that gap — a written symptom timeline, the medication list copied off the bottles rather than recalled, and the questions you meant to ask before the room made you forget them.
What the vet visit forms look like
One form is published under vet visit: Vet Visit Prep Sheet. It runs to seven titled blocks — pet and appointment, symptom log, eating, drinking and toileting, current medications and supplements, questions for the vet, weight log and notes from the visit — built from eight fields.
Those fields are three of tables to fill in, two of short labelled boxes, two of ruled writing space and one of tick-box checklists, giving 29 ruled rows in all and 10 printed items to tick or date. That mix is what decides whether a sheet gets filled in: a form that asks for prose where a tick would do gets abandoned halfway down, and a grid with no room to write a note gets ignored the first time something does not fit the columns.

Why vet visit are worth printing

The practical notes that print on the form itself run to five points; the first is: Photograph or film anything intermittent — a limp, a tremor, a cough. A ten-second video is worth a paragraph of description, and the behavior rarely repeats in the exam room.
