What is in the library

Five printable forms are published, across five families: vet visit, medication trackers, foster & rescue intake, puppy milestones and feeding logs. Between them they come to 38 titled blocks and 47 fields — 16 of tables to fill in, 12 of ruled writing space, 10 of short labelled boxes, 8 of tick-box rows with columns beside them and 1 of tick-box checklists.

That works out at 169 ruled rows and 99 printed items to tick or date. The mix is what decides whether a form gets filled in at all: one that asks for prose where a tick would do is abandoned halfway down, and a grid with no room for a note is ignored the first time something does not fit the columns.

How these are meant to be used

Every form prints on US Letter or A4 in black ink, on one or two sides, and needs no account to download. Each carries its own instructions on the sheet — 31 numbered steps across the library — so nothing has to be read on this page first and a form handed to somebody else still makes sense without you standing next to them.

The forms exist because the useful version of a record is the one that was actually written down at the time. A vet has ten or fifteen minutes and no memory of the last three weeks in your house; a sitter needs to know what normal looks like before they can spot that it is not. Both problems are solved by paper on a fridge rather than by remembering harder.
