Not veterinary advice

PetCareForms publishes reference material and blank forms for people who look after animals. It is not veterinary advice, it does not create a veterinarian-client-patient relationship, and it is not a substitute for an examination by a veterinarian who has seen your animal.

Nothing on this site states a medication dose, and nothing here should be used to decide whether to give, change, or withhold a treatment. Those decisions belong with your veterinary practice. If your animal is in distress, contact a veterinarian rather than reading further.

Using the printables

You may download, print, and distribute the forms on this site for personal use, use in a veterinary practice or shelter, and use in fostering or rescue work — at no cost and without asking permission. Print as many copies as you need.

You may not resell the forms, bundle them into a paid product, or republish them on another website. Link to the page instead. Attribution is appreciated but not required.

The sitter binder generator

Documents you create with the generator are yours. Use them, print them, and hand them to whoever needs them, on the same terms as the rest of the site.

The emergency treatment authorisation the generator produces is a written record of your own instructions and a spending limit. Veterinary practices commonly accept a signed authorisation of this kind, but they are under no obligation to, and it does not create a legal relationship between your caregiver and the practice. It has not been reviewed by a lawyer and it is not legal advice. For commercial boarding, a long absence, or an animal likely to need major treatment, speak to your practice directly as well.

The generator is provided free and its availability is not guaranteed.

Recall summaries and original sources

Recall pages are summaries of public FDA announcements, written to be acted on. Every one links to the announcement it was written from, and we do not claim ownership of the underlying notice.

Recalls are frequently expanded after the first announcement. The summary describes what was announced on the date stated; the linked FDA page is authoritative for the current scope. If a summary and the FDA announcement disagree, the FDA announcement is correct and we would be glad to be told.

Using site content

You may read, share, and link to anything on this site. Brief quotations with a link back to the original page are fine. You may not copy full pages, scrape content systematically, or republish it elsewhere without written permission.

Accuracy

Content on this site is written against a fixed specification and quality rules, and we make a genuine effort to keep it accurate. It is nonetheless provided as-is, without warranty of fitness for any particular purpose. Breed care schedules describe what is typical, not what your animal needs; parasite risk in particular varies by region and season.

Check a form before you rely on it. If you find an error, tell us and it will be corrected or removed.

Liability

To the extent permitted by law, PetCareForms is not liable for any loss arising from use of this site or its materials. Nothing here excludes liability that cannot lawfully be excluded.
