Especially worth writing about
- A factual error. Tell us the page and what is wrong. Anything clinical gets checked and corrected or taken down the same week.
- A recall we have missed. Send the FDA announcement link. The feed is checked daily but it only carries a rolling window, so gaps are possible.
- A form that should exist. What you are trying to record, and what you use instead today.
- A breed we have not covered. Ten are published; the list grows from what people ask for.
What we cannot help with
We cannot advise on a specific animal — not a symptom, not a dose, not whether something needs seeing. PetCareForms has not examined your pet and is not a veterinary practice. If your animal is unwell, call your vet; if it is out of hours and you are worried, call the emergency practice rather than waiting for a reply here.
What is already here, before you ask for it
21 breeds have a care calendar: Beagle, Bulldog, Dachshund, French Bulldog, German Shepherd, Golden Retriever, Labrador Retriever, Pomeranian, Poodle, Rottweiler and Yorkshire Terrier on the dog side, and Abyssinian, Bengal, British Shorthair, Maine Coon, Persian, Ragdoll, Russian Blue, Scottish Fold, Siamese and Sphynx on the cat side. If yours is not on that list it is a reasonable thing to ask for — a breed page is built from a fixed set of fields, so adding one is a question of research rather than of design.
Five forms are published, and five of the five families currently hold exactly one — vet visit, medication trackers, foster & rescue intake, puppy milestones and feeding logs. Those are the families most likely to gain a second form, so a request for one of them lands somewhere that is already moving.
Corrections are the most useful thing you can send
A care calendar that says the wrong cadence for a breed, a form with a field nobody can fill in, a recall write-up that has been superseded by an expanded notice — those are worth more than a compliment, and they get acted on rather than filed. Recalls in particular are frequently widened after the first announcement, and this site carries five of them; if you spot one that has moved on, say so.
The address is assembled in your browser rather than printed in this page's HTML, which keeps it away from the harvesters that scrape addresses by pattern-matching served markup. There is no contact form, because a form means fields and fields mean storing what you typed, and the privacy policy says this site holds nothing about its visitors.
